Background
There is much evidence to suggest that there has been limited work looking at developing a collaborative approach to care within forensic mental health care settings. A computer assisted structured communication approach (DIALOG) has been developed (Priebe et al, 2007) placing the user’s perspective of their care at the heart of discussions between users and clinicians. The effectiveness of the approach in community mental health services has been demonstrated but no trial taken place in a secure psychiatric setting.

Aims
This pilot study evaluates a six-month intervention combining DIALOG with principles of Solution Focused Therapy (SFT) on quality of life in medium secure settings.

Methods
A cluster randomized controlled trial design was employed for a 36 months pilot study.
Participants were recruited from six medium secure in–patient services with 54 participants in the intervention group and 54 in the control group. The intervention used a structured communication approach comprising of six meetings between user and nurse over held once a month a six month period. During each meeting user’s rated their satisfaction with a range of life and treatment domains with responses displayed on an ipad tablet. The rating was followed by a discussion on how to improve the current situation in those domains identified by the user. Assessments took place prior to the intervention (baseline), at 6 months (post intervention) and at 12 months (follow-up).
Results
The presentation will detail the results of the study and for the outcomes of interest (quality of life, levels of satisfaction, disturbance, ward climate, and engagement with services) at each time point. The aims of the pilot trial were to establish the feasibility of the trial design and to determine the variability of the outcomes of interest. Because of this the statistical analysis focused on estimating the intra-cluster correlation coefficient (ICC) from this study using standard analysis of variance techniques. A qualitative analysis examining the participants’ experiences will also be detailed.

Conclusion
The main findings from the study will be noted as well as a brief discussion as to the viability of a full-scale trial.
